This form is to stop your monthly subscription with Japanese Australia.
You must submit this form at least 7 days before the date you wish to stop your lesson to avoid being charged a lesson fee.

1. I am ceasing my monthly subscription with Japanese Australia. Failure to do so will incur a fee for all lessons that is scheduled within 7 days from the submitted date.

2. I will no longer have access to my student account and study history. I'll have to reenrol as a new student shall I wish to return to Japanese Australia.
